A photocopied page (numbered 134) from the execution register of the former Pretoria Central Prison. The upper portion is a tabular entry recording, under the columns Box No, File No, Surname and Initials, Date of Execution, Date of Sentence, Grave No and Cemetery, the following: Box B275, File V963, "MKHISE P", 1967/09/01, 1966/12/02, with the Grave No and Cemetery columns left blank. The lower portion reproduces the prison identification photograph of the man named in the entry, with two placards reading "PN No V963" and "FPN No 261844/66".
The entry documents the surname misspelling carried in the official record: although Petros Sipho Sibusiso Mkhize spelled his own surname "Mkhize", the state recorded him as "Mkhise". The blank Grave No and Cemetery columns reflect that the burial location was never disclosed to the family. This single photocopied page is the only official government record the family has been able to obtain; access to the complete prison file has never been granted.
